Health Requirements for School Entry

State of Illinois Physical Forms and proof of immunizations MUST be turned in prior to the first day of school for students that are new to the district and students who are starting kindergarten and sixth grade. Students who do not have all of the required forms on file will be unable to attend school until all required forms have been submitted.

Immunizations

  • All students entering grades 6-8 will be required to show proof of recent Meningococcal conjugate vaccination (MCV). All students in grades 6-8 must have 1 dose at school entry. 
  • Children entering school at any grade level (K-8) shall show proof of having received 2 doses of live virus Measles, Mumps, and Rubella vaccines, the first dose on or after the first birthday and the second dose no less than four weeks (28 days) after the first dose, or other proof of immunity.
  • Children entering school at any grade level (Pre-K) shall show proof of having received three doses of Hepatitis B Vaccine.
  • Children entering grades Kindergarten, 1, and 2 must have four or more doses of Polio and DTP/ Dtap vaccine, with the fourth dose given on or after the 4th birthday. Children entering grades 3-8 must have three or more doses of Polio and DTP/Dtap vaccine, with the third dose given on or after the 4th birthday.
  • Children entering school at any grade level (Kindergarten-8) must show proof of having received 2 doses of Varicella (chickenpox vaccine) or statement from physician or health care provider verifying disease history.
  • All students entering grades 6-8 must show proof of receiving one dose of Tdap vaccine (Tetanus, diphtheria, acellular pertussis) regardless of interval since last Dtap, DT or Td dose.

Annual Vision and Hearing Screenings

Vision and hearing screenings are performed annually to D34 students by a certified vision and hearing technician. Vision screening is not a substitute for a complete eye and vision evaluation by an eye doctor or an exam by an audiologist. 
 
Students are not required to undergo this vision screening if an optometrist or ophthalmologist has completed and signed a report form indicating that an examination has been administered within the previous twelve months and the evaluation is on file at the school. If a vision or hearing examination report form is not on file at the school for a student in a mandated grade or group, they will be screened.
 
Vision and Hearing screenings are mandated by Illinois on an annual basis for the following students:
 
Vision Screenings
  • Preschool, Kindergarten, 2nd, and 8th grade
  • New students to the district
  • Students receiving Special Education services
 
Hearing Screenings
  • Preschool, Kindergarten, 1st, 2nd, and 3rd grade
  • New students to the district
  • Students receiving Special Education services
