2-9-12      All Second Graders are required by law to have a dental examination. If your child hasn't seen the dentist, please make the appointment. I can send the State form home with your student for you to take along. Call me if your child has already had an examination.

2-2-12        Letters were sent home reminding parents that their child had failed their vision screening and need to have further evaluation by a professional. Please take the referral along to the appointment for the doctor to complete and return to school to be part of the Medical Folder.  

10-28-11    The rescreens have been started this week. If a student has failed the 2nd vision or hearing test, I will send a referral in the mail. This means there is a need for further testing and your student needs to be seen by a professional. Please take the referral to the appointment and have it completed and return to school as soon as possible. Call me if you have any questions about the results.

REMINDER: All CLASSROOMS ARE PEANUT AND TREE-NUT FREE.

10-13-11   October 17th, Monday, Vision and Hearing screenings will begin. ALL 2ND GRADERS WILL HAVE BOTH. ALL 3RD GRADERS WILL HAVE HEARING ONLY. ALL STUDENTS WHO HAVE AN IEP OR SPECIAL ED WILL HAVE BOTH. If I am unable to finish, I will do so on the following Monday.

If a student fails a section, I will re-screen 10-14 days later to recheck. If that area is still weak, I will send a referral in the mail. This means  there is a need for further testing. Please take the referral to the eye appointment for the doctor to complete. This is to be returned to school to be a part of your child's medical records.

9-30-11 The dead line is quickly approaching! Students who have transferred to Petty from out-of-state, or attending an Illinois school for the first time or have missing mandatory medical info, have until October 14th to comply with the State requirements. 

Students Will be EXCLUDED FROM SCHOOL OCTOBER 17 until they become compliant. I have sent a reminder in the mail and left a message at home. Please call me at 838-8111 if there is a future appointment or if you have any questions.

9-16-11 Welcome back to a new school year and welcome to all the new 2nd Graders. Let's make this year a healthy one! I know how busy this time is for everyone. I am checking all files to be sure that everyone has their required state health forms in.
